Hey guys,

This may be by design. Consider this. With the exception of Braille screen 
input, everyone the world over uses their iDevice with the screen facing them. 
When you tilt the iDevice away from you, VoiceOver says, "Landscape mode, Home 
button to the left." This is because if the screen were facing you in this 
orientation, the Home button would, in fact, be to the left. If you put the 
device in tabletop mode at this point, the device still thinks the Home button 
is to the left. Therefore, dots 1, 2, and 3 are closer to the Home button, 
while dots 4, 5, and 6 are closer to the other end of the device.

If you do the workaround by starting with the screen facing away from you, then 
going into tabletop mode, then tilting the screen toward you, and finally, back 
into tabletop mode, the tilting toward you has reoriented landscape mode with 
the Home button to the right. That's what causes the dots to orient "correctly" 
in tabletop mode.
