Well,
Changing the client preferences reduced the key repeat problems we were
seeing with the Enterprise Edition 4.4 of RealVNC in some cases but not in
other ones.  The erratic key repeats did not disappear but were less
annoying.
Also, changing repeat settings on the Xvnc side did not make any difference
in our particular case.
However, the one thing which did resolve the problems is the beta version of
an upcoming RealVNC release, to be announced.
