There extremely rarely is a two 511keV e+/e- decay signal seen. Its a special case that can only happen in a contained case. Free e+/e- annihilation usually is a 3 photon decay with <<0.01% being of 511keV...
The 511keV decay signal is an old illusion, that triggered a completely wrong understanding of physics.
