2)In Miami, you've got Online detection via "CD" (carrier detect line)
  turned on (in the modem settings), right?

It sounds to me like Miami thinks that your modem is already online
when you start it up.  That would require a "YES" answer to both
of the above questions.  It would also require that your modem be
reporting that the carrier signal is always present.  This happens
with the "&C0" setting internally in the modem.  If all these
conditions are met, Miami immediately tries to PPP communicate
with your ISP rather than trying to dial.  Since the modem is
not connected, its normal action is to echo everything back to
the computer it's connected to.  Hence, the PPP looped back error.

To solve this, make sure that your modem initialization string in
Miami contains the characters "&C1" (without the quotes).  Also,
make sure that if you have Miami set to use an exit string for
the modem, that you have those characters in the exit string as
well.  Next, quit Miami and load a terminal program, like Term.
Type the following commands directly into the modem:
AT &C1
AT &W0
AT &W1

The first one sets the carrier detect of your modem to follow
the status of the remote modem carrier.  The next two save that
setting in both non-volatile RAM areas.

After doing all the above things, give Miami a try again and
see if it works right.
