How about using Appleworks?

In the word processor just do you normal (right way up) text for the questions.

To create the upside down answers hold the Option key whilst dragging the mouse. This will create a text box. Enter the answers in the text box. Select the rotate tool and rotate 180°. Easy.

If you want to embed the graphics block into the text cut it and select the Text tool and paste it, this makes the text block move with the normal text.

I can't find this feature in Word X for Mac (1983-2001) or the old BBEdit Lite.
