There's a perfectly good reason why Andy refers to Miss Crump as Miss Crump: that was her name. What else would he call her? Teach? Old Lady Crump? Hey You?
The character of Miss Crump first appeared in Andy Discovers America, but she would not get a first name until six episodes later in A Wife for Andy. Even after arranging a dinner with Andy and Miss Crump, Barney still didn't know her first name. The next day when Andy says "Helen," Barney doesn't know who he's talking about.
