What are TPAs doing to ensure their own employees' health claims are kept secure (and 
only accessed when minimally necessary)?

I'm interested in finding out if there is an "industry trend" or generally accepted 
way of dealing with in-house PHI. Are employee claims funneled to specific examiners? 
Are employee claims out-sourced? Are policies enough?
