I assume you mean the options like home, instant recovery, settings, snapshots, task scheduler, etc?

If you look in the user's guide, you'll see the tables containing all the navigation commands for the new layout if using the NVDA 2013.1 beta, or the NVDA snapshots. This is in section 5.4, Object Navigation.

I use the desktop keys which seem to work while using the laptop layout also, but I've tried with the assigned object navigation keys for the desktop as well with either layout. You need to hold down the NVDA key, then use numpad 4 and 6 to move between objects, NVDA-numpad 8 to move to objects containing current navigator object, and NVDA-2 to move to first contained object. Using the numpad keys, I find it makes more sense and is easier to figure out, but this seems to work whether or not you're using the laptop or desktop layout, so it really doesn't seem to matter much. If you prefer, you can switch between the two key command sets on the fly without even switching layout, apparently.

When you move the review cursor, you can press NVDA+numpad enter to activate an object, but you may need to use numpad 4 and 6 without the NVDA modifier to move to the part of the menu item you want to click.
