You don't have to use a C# UI there is also a much more standard UI - WixStandardBootstrapperApplication http://wix.sourceforge.net/manual-wix3/wixstdba_intro.htm. From what I remember of screen readers (admittedly based on work I did 20 years ago) I think they should be compatible.
