If that log extract is correct (and verbose), the custom action isn't actually in the MSI file. If it failed or wasn't called because of the condition there would be something in the log. I'd verify that it really is there, using Orca.
Incidentally, after CostFinalize is too late because CostFinalize is what looks at the feature conditions.
