there are a few related behaviours i have observed that may be related... i
have the 'manually place window' option enabled, so if a window opens up (eg
pidgin chat), i see the window outline and have to click to place it on the
screen.

however, if the window is a splashscreen (eg openoffice) or a dialog, then
it will just appear on the screen.
but some programs just appear anyway, like kpdf

also, i restore my firefox profile that has maybe 6 windows, each with about
10 tabs.  some of those tabs have username/password prompts appear (eg
basic-auth).   i can click-to-place maybe 3 or 4 of the firefox windows as
they appear, but the last few seem to just put themselves on the screen, and
i don't get a chance to click-place them.
