I use Photoshop. I will list my usual steps below:

1) Pen and paper, mock up ideas and notes
2) Get into Photoshop, work on the general layout
3) Back to the pen and paper, drawing the layout in pencil and breaking up
the layout into there elements.
4) Back into photoshop, break down and export the graphics needed
5) Create the Plain HTML page with all its content
6) Style the document using CSS
7) Test Test Test and Revise as needed

This is usually the way i work, i find it easy breaking down the layout on
paper, its alot quicker with a pencil
and ruler and you can sit the peice of paper on your desk when your marking
up the HTML and CSS.
