An <?xml ?> declaration (or anything else before the Doctype) will
trigger Quirks mode in IE6 so should be avoided.

The prolog is optional if the defaults (XML 1.0 and UTF-8 or UTF-16) are
used.

It is forbidden if HTML 4.01 is used.
