I am at a loss as to what I could put on the page that would explain
this without adding lots of words.  The text field labelled "other" is
in square brackets '[...]' and follows a drop-down box all on the same
line where it says "host" (or "path" etc).  What else can it be except
and alternative for the entries that exist in the drop-down list.  And
when the user investigates the drop-down list they see the description
of what "other" means.

If you changed the URL on the first page then the URL for the second
page would have changed.  It is therefore apparent that you failed to
make the change.  Having seen that the URL is unchanged it would have
been wise for you to go back and investigate.

I agree, at the time I couldn't work out a way of making it work so
that if there was an entry selected (e.g. by selecting "edit selected"
and pressing "Make Change") it did default to replacing the selected
one.  I have now worked out a way that seems to work.  It will now
default to "insert entry before selected one".

No, not selecting anything is very different from selecting everything.

You pressed the button where it is clearly labelled "Insert the value
above after the entry selected below" yet you failed to select an
entry below.  It tells you in the option that you chose that an entry
must be selected from below yet you failed to do it.

I have also made this default to selecting the first entry.

You only need to clean out the "other" boxes if you choose "other ..."
from the drop down list.  Choosing anything else from the dropdown
lists will make a change.
