Without commenting on the rest of the proposal, "QName" should be
changed to "UniversalName" or a similar name. In the XML namespaces
spec, QName refers to a prefixed name (e.g. foo:bar). No term is
formally defined for a URI/local name pair, but the spec does note that
they are universally unique.
