iirc, gnuboy (a gameboy emulator) can use the Xv extension, however it doesn't use it directly. The SDL port of gnuboy supports "hardware YUV scaling with full interpolation on systems that support it," and it's SDL that uses the Xv extension for the hardware scaling.
