This has been a problem I have had as well.  I mostly use mame in an arcade 
cabinet.  That means I have to navigate with a trackball or joystick.  Most 
of the frontends are really geared at using the mouse.  I have found flynn 
which works well enough with the joystick, but is not practical for more that 
about 60 games at once.

When I get frustrated enough I will probably start working on my own frontend 
geared at working with the trackball( it will not use a cursor ).
