I'm sorry but this has never, ever, happened to us, despite the fact we 
very often use <filename>.

Technically speaking, given the way XXE has been implemented, I don't 
see how this could happen. Therefore I'm almost certain this is not an 
XXE bug.

May be, from time to time, some of your authors inadvertently set this 
attribute.
