In case you can't find a card that will do so, Matrox cards have
always been a snap to get working with sync on green and composite sync
for me, I have had 4 or 5 fixed frequency monitors that I obtained
for free that worked great with Millenium I's and Mystiques.  Just
add a 

 Option  "Sync_on_Green"

to your device section in your XF86Config file.

I have seen methods of building your own frequency stabilizing circuit of
sorts using off the shelf stuff from radio shack that can allow you to 
see even the bios screen if it's a big concern, but that may be just as much
hassle as buying a card that is specifically for your fixed freq monitor.
