I would say all of the above. The top twitch streamers don't make a fortune, but it often leads to appearances in other gaming events that they get paid for.

There isn't a lot to understand if you just view it like a job. How do people work in a factory for 12 hours? If you're popular on twitch, you can definitely make more than minimum wage, and you get to play video games all day.

I don't watch it, and I don't really understand why it's interesting to people, but it is..

And how does someone stream their game play for 12 hours straight? Astronaut diapers? Lots of Mountain Dew and Doritos? Or do they get breaks?
