Looks like a hardware problem. Is there anything else I can check before
coming to this conclusion?

No.
What happens if you stick an Amanda tape in the drive and do this:
$ mt -f /dev/nst0 rewind
$ dd if=/dev/nst0 bs=32k
You should see the (ASCII) Amanda label.
