I found some old references (10/2005) to this same issue, but I've not been able to resolve for me yet.
Andrew Baxter asked for clarification "I would like to know what you are using as your RequestID field". The issue I'm finding seems to be related to the fact that my RequestID field is more than 15 characters. My problem is that I have nothing unique that is shorter than 15 characters.
