Lei, We faced the same issue. On closer look we discovered that on SRM:Survey form submitter field is set to the support person's loginid who Resolved the ticket when it should have been requester's loginid . You might want to check value in submitter field if it is set to requester Login id. If not you will have to modify the workflow suitably. There is one filter on SRM:Request form which creates surveys -I dont remember the name- you can modify it to get desired result. Hope that helps!
