not sure if this helps, but for tomcat, the users are stored in file 
tomcat-users.xml on the server.
The file is located under $tomcat_install\conf directory.
 
For mid-tier, I think the admin password is stored in the config.properties 
file (encrypted) under $mid-tier install$\WEB-INF\classes.
You may want to try to copy the encrypted password string from another mid-tier 
where you know the password. It may work.

I believe the default password for Midtier is administrator.  I am not sure if 
you set the password for Servlet Exec or if it sets a default password.  I 
would go ahead and try the good ole stand by's:  administrator or admin.  It is 
worth a shot before resorting to reinstalling.
