Have you recently been able to have the DB bounced and restart the ARS service?
It sounds like you are either experiencing database corruption or ARS is all mixed up. I'd try bouncing the DB and restarting ARS before doing anything else. If your items are still missing then you most likely will have to do some manual DB things.
