Turns out that we have two different BMC Remedy Action Request System Release Notes for 7.6.04. Both are dated 1/28/2011 and have the same title but the one with this tidbit about the arsystem.tag file has a document number 184374 and the shorter one without is document 180022.
That still leaves the entry violates a unique index and creation of an application form failed errors (ARERR 382 and ARERR 4550 respectively). Anybody have any ideas?
