What this tells me is that the import of some of the forms - those listed with the ARERR 55 - failed because they depended in some way on fields that were not present on other forms. For example the CHG:ChangeInterface form is missing field id 1000003216. This form is a self join of CHG:Infrastructure Change so it looks as though it's missing on that form. An earlier stage of the installation should have imported this so that it is present. Why it didn't I can't say from the snippet below. I think your best bet at this point is to open an issue with support and get them to look at the full set of logs you have as it's going to take some back tracking through them to work out where the first problem occurred - what we have below is a subsequent failure.
