Interesting that after uninstalling ActiveState, renaming the directory and 
rebooting then reinstall ActiveState all my Repositories are still there.
I have all of yours except but roth.net, but I also have:
http://www.bribes.org/perl/ppm/packages.xml
file:///C:/ASSP/assp.mod/


Does it make any difference that my preferred Area is set to C:/Perl/site 
rather than C:/Perl?

Repositories must be added (edit, preferences, repositories using the ppm 
GUI) to the default activestate ppm installation to find some packages.

I have 4 additions:

http://www.roth.net/perl/packages
http://ppm.tcool.org/archives/package.xml

www.roth.net might be down right now, though.
