Here is what I normally do

1. Install Cent OS Basic Core
2. Install mysql & apache
3.Install Zaptel,Asterisk,Asterisk Addons
4.Install FreePbx
5.Always there is a x100p card in every of my box
6.I always use the virutal pri
7.Large scale implementation. I never place the box in house. use the
colocation facilities.

I want to share with one more thing ... I never have single problem with
this type of configuration.No reboot.
FreePbx forum is the great place to look the more advanced features and
custom modifications.
