More info on the sip.conf file is needed and exact details (minus
password) as-is in the GUI is required to give you a proper analysis.
On some of the older phones, to the best of my recollection, "Phone
Number = Username ". If you have the phone number blank, then the
phone tries to authenticate the password based on an empty username.
You have also not provided us the model number of the phone and firmware
version, which could very well be relevant in gearing you towards the
right direction.

Ok, I am not a fan of Polycom phones, but I have to deal with some.
I need to have a phone point 1 "line" to Asterisk Server A and "line" 2 point
to Asterisk server B.
Right now it always seems to authenticate to the first server only.
On the phone GUI, both line have the same extension but different passwords. On
the asterisk servers I only see the phone attempting registering on server A:
