I second the Vitelity service.  their support is usually VERY quick.. the
rates are reasonable and over the past year I have found the service to be
very reliable here in the USA.

I've ported to them with no problem, the web based account interface is good
at walking you through the process. Be ready for a lot of conflicting
reports though with most providers that are on the wholesale pricing side of
things.
