I would say it depends on the game or even what you are doing in said game. Manamon 2 seems to be a pretty good example, battles are smoother with a screen reader since you can interrupt more stuff than with SAPI, but the tutorials in the Music Hall are much harder to follow with a screen reader than with SAPI. Now perhaps the game could have been coded so that both issues could have been avoided but given that I'm hardly a coder myself, I won't insist too much on that. Still, here is an example of a game where the user has the choice between Screen Reader and SAPI and having the choice is neet, for the reasons I mentionned.
On the other hand, I didn't try Entombed with a screen reader, but I would guess the battles would sound a bit messy, just a supposition though.
