1: Yes, mostly. Typically users connect to their raspberry pi's with an SSH client or over wifi, though you can also install command line TTS, there's also guides for a bit of a desktop hack with Orca, though i'm uncertain if that will work with the newer Pi 4.
2: There's a guide [here] on how to install the OS and setup the SSH and wifi settings, bt you'll need a MicroSD card to install it on to. Simply plug it into a computer, not the Pi, and install and setup everything, then pop it into the Pi and your good to go. By connecting over SSH or wifi with your computer, you can use a regular screen reader to read the output.
3: Raspbian is the standard operating system for Raspberry Pi's, though you could also use things like Debian, Windows 10 IoT, or Arch Linux, among others. The guide uses Raspbian Buster Lite,not sure if that may present any issues with current setups.
