The guide uses the Headless version of Raspberry Buster Lite, which by default only uses the command line and has no desktop. There is a specific reason for this, as the desktop editions of raspbian are not natively screen reader accessible. As I said, i'm not sure if previous guides for setting up a workaround with Orca will work with the newer Raspbian Buster, or whether a new solution could be worked out. If your interested in a version of Raspbian Buster that has a desktop environment, or other supported OS's though, you can check out the downloads sections of the official Raspberry Pi site [here], you'll also find the Raspbian Buster Lite edition there, which can be found [here].
You can also find Windows 10 IoT Core there, though i'm not sure if it has Narrator available. It's also limited to the UWP platform for windows. Sadly, NVDA doesn't have an arm based distro at the moment that i'm aware of.
