I use a free and open source program called open broadcaster software, found at https://obsproject.com/
You can record videos, as well as stream if you like, plus it captures your mic's audio along with the computer's sound.
It is pretty much accessible and works with NVDA (not sure about other screen readers but I'm sure it will work). Just ask if you need any help with setting it up.
