I believe the problem could be that you wrote eof to the drive using the
non-rewinding device, unless you issued a rewind separately.
However, the tape being unlabeled doesn't entirely fit that theory.

Just in case, here is the string I use to wipe a tape:
sudo mt -f /dev/st0 eof

Are you sure the tape's write protect flag is disengaged?

If none of my suggestions help, please reply with the version of bacula you
are using. Also, how many drives do you have? Is it possible the label
command is referencing the wrong drive?
