I just had success with this, with the point and mark in the
*BBDB* buffer:

   C-u C-o phone RET work RET +33 1 42 45 09 16 RET

to input a French telephone number, despite having
...north-american-phone-... set t.  That looks like a workaround
for you until you find what setting that variable incorrectly for
you.
