I use bbdb-expire which deletes 1. Not used bbdb address after a period of your choice, 2. Allows you to add to each entry a persistent field, which protects it from the automatic deletion. I also use gnus splitting on my imap server for dealing with spam, based on the entries in by bbdb. So spamers go automatically into a SPAM folder on the imap server from which they are only added to my bbdb database if I save this message to another folder.
