Apparently, Bruce forgot to put in the exception about package names.

Here's the text Bruce wrote:

"My thought is to use the developer's capitalization *everywhere* when
referring to package or package component proper names, even when we use
them to start a sentence, index entry, or TOC entry."

(emphasis mine)

I really think it is wrong to change the capitalization the maintainer
provides, simply because it is used in a title.

I would like clarification on this.
