Your best bet on windows will be to run nethack under the windows command prompt and turn off all the graphics in nethack. That way you get an ascii display your screen reader can handle properly. After that, it’s time to learn symbol meanings. Accessibility work needs doing for the graphical nethack to work in windows and the developers could install nvda on a machine and try it out with nvda in graphical environment to discover what work needs doing.
