Did you configure this via tftp or manually?
I'm asking because you've got this: ccm-manager config server x.x.x.x

Can you do:
*debug ccm-manager config-download all

*then do *
no ccm-manager config
ccm-manager config*

And see if you are being able to download the config. paste the output
here.

As well you can attach the relevant pages of CCM configuration.
