My first thought is termination. And you would probably not know if CD
reads failed, since they are going to be retried in most cases. The
Adaptek is rumored to be fussy about correct termination, and based on
one problem I had it is.
If you are using termination in the last device on the cable, be sure
the other devices are not trying to terminate, be sure there is no
external terminator, etc. All the stuff you probably checked already.
See what the controller says about termination in 'dmesg' output, if it
says it is turning on termination in the controller or some such it
probably doesn't think you are terminated.
