First, do what you love as long as you can be happy with what you're making doing it. If that's healthcare, stay put, if not, go for e-commerce. One thing with e-commerce though, the banks and merchant processors are getting really cranky about PCI compliance. Even for small merchants you can self-certify but if there is a breach it will come back to haunt you in a big way. Make sure you're comfortable with the PCI compliance rules and get current with secure coding methods before you start building those kinds of applications.
