Seems like a good round up of all sorts of failures. As one would guess, there is plenty of blame to go around amongst both the private sector and the government. The one thing that really amazes me as I've read through some of these reports is that no one ever seems to have had to answer the question, "what do we do if something really bad happens". I'm amazed that these catastrophic failures don't happen more often.
