Robert,
Before you pin an ADD label on him, ask your doctor about doing a
double-blind study with him. My mom used to do counseling with kids, and she
would often see kids that were part of double-blind studies to determine if
they really needed ADD drugs. If they need them, it's quite apparent when
they're on the actual drugs and when they're on the placebo. (Though, it
sounds like he's already been on them and it didn't make an improvement.)

You might also want to consider some behavior modification techniques. One
of our coworkers has an autistic son and they do "social stories" with him -
essentially cartoons that they draw about what kinds of situations he is
likely to encounter and how he might behave in those scenarios. It helps
prepare him for things. So, in your case, you might draw a social story
about riding the bus - what the bus driver might ask him to do, how he
should respond, etc.
