I have at one time or another experienced all three.  The end result was as
follows:

1.  Humana - the most paperwork, the most delays.

2. Aetna - straight forward, least amount of paperwork, smallest formulary,
smallest co-payments.

3. Blue Cross/ Blue Shield -  smallest choice of approved vendors, most
expensive in terms of premiums and co-pays.

I usually choose Aetna if I have the choice.
