I'm not sure this would be the issue, but wouldn't be surprised if it is (it's a Windows ISA server/router).
The problem started when we changed our login routines to our web app (it's NOT using system logins - our app uses a database for user info, and CF code for authentication). However, other unrelated apps are also affected by this - we get 404's there as well, but then a simple refresh finds the page no problem. So, I'll be looking into the router issue (shuddering at trying to figure out the ISA server's bastardised routing standards - I'd prefer if it were a Cisco router, something I know)....
