When I ran IT for Seacrets we used SurferNetwork. They hosted the video 
(can handle a variety of formats) and handled all bandwidth, we placed 
the links in the code. Only paid for what we used (which was a lot), and 
  had a fantastic deal. Full reporting on usage by file. Never once had 
a problem.
