I host on Linode w/ these specs:

i686 GNU/Linux
Ubuntu 10.04.4 LTS

I do regular apt-get update/upgrade, so it's currently up to date.
This is the first time I've had a problem after the upgrade.

Here's my /etc/hosts file (w/ public server name and IP's changed):

127.0.0.1            localhost.localdomain    localhost
111.111.111.111            sub.domain.com    sub
28e6:28e6::28e6:28e6:28e6:28e6    sub.domain.com    sub

# The following lines are desirable for IPv6 capable hosts
::1     ip6-localhost ip6-loopback
fe00::0 ip6-localnet
ff00::0 ip6-mcastprefix
ff02::1 ip6-allnodes
ff02::2 ip6-allrouters
