     Try www.tinypawsco.com under "beds-sleep bag".  Gracie has one in pink daisy fleece which reverses to solid fleece.  It's very thick and just a little bigger than a standard size pillow case, but I think you select the dimensions and I know you select your fabric choices.  Gracie hasn't slept inside it yet because, well, she has my hole bed to sleep in, lol.  Still, I plan on putting it in her playpen come the fall/winter to keep her warm.  :)  Because the fabric is so thick, you can actually make a nest or soft bed by squishing down the sides.
     If you want to go high end, try www.gwlittle.com for the super plush convertible bags that can be laid flat or turned into a pouch.  I forget what they're called, they're beautiful, but expensive.
